Notice of Privacy Practices

Parkview Counseling is required by law, including the Health Insurance Portability and Accountability Act (HIPAA), to maintain the privacy of your protected health information (PHI), to provide you with this notice of our legal duties and privacy practices, and to notify you promptly if a breach occurs that may have compromised your information.

Everything you share in therapy is confidential. We do not sell, rent, or share your information for marketing purposes, ever.

We may use or disclose your health information without written authorization for:

• Treatment — coordinating your care with your therapist or, with your involvement, other providers.
• Payment — billing your insurance plan or collecting fees for services provided.
• Health care operations — quality improvement, supervision, licensing, and the everyday running of the practice.

We may also disclose information when required by law — for example, to report suspected abuse or neglect of a child or vulnerable adult, to respond to a court order, or to prevent a serious and imminent threat to someone’s safety.

You have the right to:

• Request a copy of your health record, in paper or electronic form.
• Ask us to correct information you believe is inaccurate or incomplete.
• Request confidential communication — for example, that we contact you only at a certain phone number or address.
• Ask us to limit what we use or share, though we are not always required to agree.
• Receive a list of certain disclosures we have made of your information.
• Receive a paper copy of this notice at any time, even if you agreed to receive it electronically.
• File a complaint if you believe your privacy rights have been violated.

We will never retaliate against you for filing a complaint.
